EXPLANATION OF FORMS
Camper Travel: This form needs to be filled out for ALL CAMPERS. This form will let us know your daughter’s travel arrangements to and from camp this summer and if she will need to use camp transportation. There are a limited number of spaces on our buses and they are filled on a first come, first served basis. Please complete this form as soon as you can. This form can be filled out online or downloaded, printed, filled out and faxed or sent back to camp. It needs to be returned to the camp office by May 1st.
Parental Input for New Campers: This form needs to be filled out for ALL NEW CAMPERS. This form will help us get to know your daughter a little bit better. This form can be filled out on line or downloaded, printed, filled out and faxed or mailed back to camp. This form needs to be returned to the camp office by May 1st.
Parental Input for Returning Campers: This form needs to be filled out for ALL RETURNING CAMPERS. This form will help us get to know your daughter a little bit better. This form can be filled out on line or downloaded, printed, filled out and faxed or mailed back to camp. This form needs to be returned to the camp office by May 1st.
Health Form: This form needs to be completed for ALL CAMPERS and returned to the camp office by May 1st. This form is not complete without the signatures of the camper’s physician and parent/guardian. As this form requires a signature it is only available in .pdf format for downloading purposes.
Walgreens Form: This form needs to be filled out if your daughter will be taking prescription medication at camp this summer.
Senior Trip Permission: This form needs to be filled out for ALL SENIORS. (Our campers who are CURRENTLY in the 9th grade.) If you would like more information on this trip, please click here for the girls’ itinerary. As this form requires a signature it is only available in .pdf format for downloading purposes. This form needs to be returned to the camp office by May 1st.
Self Administered Meds Form: This form needs to be filled out for ANY CAMPERS WHO MUST CARRY MEDICATION WITH THEM THROUGHOUT THE DAY. This would include meds such as inhalers and epi pens. If your child has medication that is to be kept in the Health Care Center this form DOES NOT have to be filled out. This is only for emergency meds that must be kept with the camper at all times and in the bunk. This form can only be downloaded, printed, filled out (signed by a doctor) and faxed or mailed back to camp. It needs to be filled out and returned to the camp office by June 1st.
Parent Travel: This form needs to be filled out for ALL CAMP FAMILIES, as needed. If you have more than one daughter at camp, you only need to fill it out once. This form will let us know where you will be this summer and how to contact you. This form can be filled out online or downloaded, printed, filled out and faxed or mailed back to camp. It needs to be filled out and returned to the camp office by May 1st.
